Dear sir:
We have been given to understand that you are planning to enter Phillips Academy this coming fall. Our records, however, do not show that you filled a formal application for admission, and I am enclosing herewith, therefore, a blank for this purpose. May I ask you kindly to fill out and return to me at once the enclosed blank? If possible, your testimonial letters should be sent at the same time.
